Answer: All CAPF 70-5 evaluations, annual or abbreviated, require a questionnaire completed and uploaded for the airplane to be flown for the evaluation.
Reference, in CAPR 70-1 under form 5 eligibility:
7.5.1.2. Complete the appropriate questionnaire (CAPFs 70-5Q-A, 70-5Q-B, or 70-5Q-G) for the make and model of CAP aircraft in which the member is being evaluated;
Also earlier in 7.3 it explicitly calls out a questionnaire as required for abbreviated CAPF 70-5s (underlines added):
"...has the discretion to administer an abbreviated Form 5 consisting of an aircraft questionnaire (CAPFs 70-5Q-A, 70-5Q-B, or 70-5Q-G) for the aircraft model to be demonstrated..."
